Sanitation & Infection-Control Procedures

I acknowledge that I had the opportunity to observe or ask questions about the studio’s sanitation and infection-control procedures used during my appointment, including preparation of the workstation and appropriate single-use or sanitized equipment.

Hand Hygiene

I acknowledge that I witnessed hand washing with use of hand sanitizer prior to putting on gloves during my appointment.

Glove Changes

I acknowledge that gloves were removed/changed and hands were sanitized routinely where needed to access additional supplies not accessible on the work station.

Aftercare

I confirm that I was provided with verbal and/or written information regarding tattoo aftercare and understand that I am responsible for following the aftercare instructions provided to me.

Healing & Infection

I understand that tattoo healing varies from person to person and that the final healed appearance may differ from the appearance of the tattoo immediately after the appointment. If I experience symptoms that concern me or suspect an infection, I understand that I should seek appropriate medical attention.

I confirm that I had an opportunity to ask questions, raise concerns, and have anything clarified before completing my appointment.
