Raffle prize agreement form

This agreement is made between:

Artist: Christine fenzl -operating as Ink Is Art Tattoo Studio
Client: signed below

Together, we’ll refer to both as “the Parties.”

1. What This Is All About

You’ve won a tattoo prize through a raffle hosted by Ink Is Art Tattoo Studio. This agreement outlines the steps required to claim your prize and ensures clarity for both you and the Artist.

2. Raffle Prize Details

Prize Value

• Your prize covers one tattoo session. The total value amount indicated in your raffle that you participated in

Design Integrity

• All Designs are to be orioriginal and cannot be replicated from someone else’s design
• Placement can be discussed, but final decisions rest with the Artist’s professional judgment.

3. Claiming Your Prize

Form Submission

• You must submit a clear and concise description of your selected design within 30 days of the raffle announcement.
• Use this form: www.inkisart.ca/christine

Payment (if applicable)

• If your raffle prize includes partial coverage (e.g., discounted tattoo), any remaining balance must be paid hourly at $150 per hour at the end of each session
• Send payment via e-transfer to: christine@inkisart.ca

4. Booking Your Appointment

Scheduling

• Once your form is submitted, the Artist will coordinate a mutually available date for your tattoo session.
• reschedule is not permitted- please plan and coordinate in your schedule to be sure you’re available for your appointment. Should you be ill the day of your appointment reschedulingis the Artist’s discretion-a no-show fee or a rescheduling fee may be required to rebook at a minimum of $150

• Multiple cancellations or no-shows will result in forfeiting the prize.

5. Artist’s Rights

Refusal of Service

• The Artist reserves the right to refuse service for reasons such as health concerns, intoxication, abusive behavior, or violation of studio policies. No prize or refund will be issued in these cases.

Creative Control

• The Artist maintains full creative control over the tattoo’s execution, including adjustments for size, placement, or skin type.

Copyright

• All flash designs and completed tattoos remain the Artist’s intellectual property. You receive the physical tattoo, but not reproduction rights.

6. No Refund Policy

Raffle prizes are non-transferable and non-refundable. If the Client forfeits the prize due to missed deadlines or cancellations, the prize will be void.

7. Health & Safety

Tattooing involves some risks, including infection, allergic reactions, or dissatisfaction with the final result. By signing, you agree not to hold the Artist or studio liable for any issues that may arise.

8. Legal Stuff

This agreement is governed by the laws of Ontario, and any disputes will be handled in Durham Region.

9. Final Notes

This document represents the full understanding between both Parties and replaces any previous conversations or agreements.

Next Steps to Claim Your Tattoo

To move forward, please complete the following within 30 days of the auction announcement:

1. Submit Your Design Choice
Visit www.inkisart.ca/christine and fill out the form with a clear description of the pre-drawn design you’ve selected. This form gives me all the details I need to prepare your tattoo.
2. Send Your Payment
E-transfer your winning bid amount to christine@inkisart.ca once your form is submitted.

Failure to complete both steps within the 30-day window will result in forfeiting your prize.
