Client intake form


Hi thanks for your interest in getting a stunning tattoo by Your’s truly

PLEASE NOTE: 1 form =1 tattoo – this form is for 1 tattoo/ 1 person only- if you have multiple tattoos or looking to book for more than yourself, each person/tattoo will be required to fill out an additional form.

as of 2022 all recipients getting a tattoo by me will be required to be 18+ 
(sorry no longer offering my services to anyone under 18 even with parental consent)

hands and/or feet are also not being done at this time.
Phone consultation/email I am not doing in person consultations currently however should you wish to meet with me privately we can arrange a time at this over the phone meeting. 

RATES: as of Jan 2023

($180 per hour – 50% required at time of booking)

1/2 day rate $550 up to 3 hours. $75 due upon booking

full day rate $1000 up to 4-6 hours $150 due upon booking

What’s included

FREE online or in person consultation

FREE – no added or hidden charges for designs, drawings changes, supplies or numbing used during session if used.

FREE initial bandages or products for optional aftercare methods such as saniderm, tattoo armour or other aftercare methods depending on what’s best for you. Additional is available for a small fee.

FREE starter aftercare pkg with adequate amount of supplies to get you started. Upgrade your aftercare pkg for a small fee

payments accepted:

Cash – preferred

Debit/Credit machine also available – e-transfer is only for booking deposit.

Currently i am booking 3-4 months from now. 

to reserve a appointment you must pay 50% of the rate for your apt.

to be sent via INSTRUCTIONS PROVIDED ONCE BOOKING IS MADE this will be deducted from tattoo cost once completed AT YOUR FINAL APPOINTMENT.

I will respond to this form via e-mail and will give a estimated quote. 

Appointments are booked by the hour and a min/max is given as a estimate

unless you wish to book full or half day. for your approximate time required for the requested tattoo, should you change your design the minimum time can not be changed after you have your confirmation booking has been made.

To ensure YOU our client respect MY/YOUR ARTIST time and efforts, THIS 50% non-refundable deposit is to confirm your appointment and time dedicated to you. After your tattoo is done, this fee is deducted from your tattoo’s final price. Should you cancel with less than 72 hours before your appointment or not show up to your appointment this payment will be forfeit. And a new 50% deposit will be required to rebook/reschedule.

Once I receive this form I will contact you via phone or email to discuss the tattoo further and we can discuss going forward at that time. 
Please allow a few days (mine are Tuesday-Saturday) to respond to this request as i am only able to call when i have time between clients i am usually tattooing 10-6 each day. Thank you Christine

Please note: due to copywrite laws etc. I do not copy or replicate tattoo designs you are submitting as reference. I will only recreate my own artwork to use for your tattoo purposes. 
if your looking for an exact replication of a tattoo you found online I will NOT do it. 
I also don’t do fingers or hand tattoos or face tattoos. This is against my policy.



If this form wont submit, its usually your photos you took are too large.

please remove the photos of yourself and try to submit- and if i need the images i will ask during our phone conversation.
